The City Clerk's Office issues certified copies of birth, death, marriage and divorce records to immediate family members for events that occurred in Concord, New Hampshire. We also have access to statewide records for births beginning with 1935 to present (except 1949 and 1950); death records 1965 to present, marriages records 1960 to present and divorce records from 1979 to present.
Statewide civil union records may also be requested through this office. For divorce records prior to 1979, please contact the courthouse where your divorce took place.
A $15 fee applies for the issuance of one certified copy of a vital record; additional copies purchased at the same time are $10 each.
Please be aware that New Hampshire law requires that a nonrefundable search fee of $15 be collected for each record requested. If the record is located and you meet eligibility requirements, you will be issued the requested number of certified copies of that record.
To request by mail please complete the attached application (PDF) to request vital records from our office. Please mail your request along with payment, make your check or money order payable to the City of Concord, and a self addressed stamped envelope to:

Please note that a valid photo ID is required in order to process your request. A legible photocopy of the applicants government issued photo ID should be submitted along with your request.
